As far as output to EFILive goes, that's another analog output you have to setup and the manual should tell you how to do it. Then you would mount the controller (in your case that PLX box) somewhere under the dash, mount the gauge, and hook it up.

These wires should be in the same loom as the wires that come down to the wideband, no biggy there. There should be analog outputs from the controller to attach to the narrowband's wiring (in order to give the ECM a reading).

the narrowband is then removed (unplugged) from the factory harness. Usually, in the case of a wideband/wideband controller, you remove one of the narrowband 02 sensors and install the wideband in it's bung in the header. Although I have never seen a PLX install, running these widebands is pretty close to a universal thing, so I'll give you some generic instructions, make sure you refer to the manual before you do anything crazy like cut up factory wires.
